Outpatient Behavioral Management Specialty Assessment
This assessment is provided in cooperation with the Illinois Department of Human Services. Children and adolescents eligible for this service are those with multiple treatment failures suffering from emotional and/or behavioral problems that interfere with daily adaptation skills. Required criteria to qualify for this assessment are:
- Must either be uninsured or have Medicaid
- 3-18 years of age
- Multiple treatment failures
- Emotional and/or behavioral problems that interfere with school, friendships, family relationships and self-esteem
- At risk of separation from family
- Must demonstrate a clinical need for the assessment
This assessment is free. This service is offered Monday through Saturday by appointment. Once the final assessment has been completed, the report is reviewed with the patient, their family/caregiver, the referral source and The Department of Human Services.
Program Components
- Interview with parent/caregiver, child and teacher
- Emotional and behavioral screening
- Psychological testing (if needed)
- Intelligence testing (if needed)
- Psychiatric /medication evaluation with child and adolescent psychiatrist
- A comprehensive written report with treatment recommendations
- Free follow up consultations
